I think that a good index adds to the value of any piece of
documentation, and I have to confess to being slightly baffled that
there's an opposing view.  If there were a way of automatically
ensuring that the documentation matched the code, whether through
automatic generation or automatic testing, that might make the
maintainer's job easier, but that may not be practical.
