Fix up headings for issue templates

The headings used to be `**foo**`, but it's better to use `## foo`.
The old headings would cause the following paragraph to be appended to
the heading.  Thus
```
## foo
Text
```
became
```
foo Text
```
with "foo" in bold.

With the new markup, this doesn't happen.
